Beginning the Program
Your first major step in beginning the Program is to complete the Lifestyle Evaluation Questionnaire provided. This consists of 33 questions about your present lifestyle. The questions are very personal and require you to be very honest and frank about the reality of things as they are with you at the moment. Your detailed responses do not need to be shared with anyone else, even your “buddy” [more about your “buddy” later]. You are encouraged to download or print out the Questionnaire so that only you know how you have answered the questions. Please note that this web site has no way of knowing how you answer the questions. When you have completed the Questionnaire, you will be required to score your answers according to the Scoring Key provided on the web site.
Each question will have the same five possible alternative answers. These are “YES” , “NO”, “SOMETIMES”, “OCCASIONALLY” and “RARELY”. You will choose one alternative answer for each question by ticking in the appropriate checkbox. After completing the Questionnaire, you will be asked to count up the number of responses you have made to each of the five possible alternatives and to record these numbers in the box provided on the questionnaire form.
